Dans la maladie d’Alzheimer, l’acupuncture peut être efficace

l'acupuncture améliore de façon statistiquement significative les résultats sur l'échelle Mini Mental State Examination (MMSE) avec une différence moyenne standardisée à modèle aléatoire (SMD) égale à 1,05 (IC 95%, 0,16 à 1,93). Cependant, il existe une grande hétérogénéité des six ECR (I² = 57% ; p=0,04).

L’utilisation de l’acupuncture pour traiter la maladie d’Alzheimer a augmenté en fréquence ces dernières années. Cette revue systématique de la littérature et la méta-analyse qui s’ensuit, a pour but d’évaluer l’efficacité et la sécurité de l’utilisation de l’acupuncture dans cette pathologie. Elle a été réalisée à partir des essais contrôlés randomisés retrouvés dans le Registre central des essais contrôlés, PubMed, Medline, Embase, PsycINFO, la littérature de biomédecine chinoise (CBM) et les bases de données médicales chinoises (CMCC et CNKI) de leur création à juin 2014. Dix essais comparatifs randomisés (ECR) pour un total de 585 participants ont été inclus dans la méta-analyse (ECR avec des populations étudiées variant de seize à quarante-neuf personnes).

La durée du traitement varie de quatre à vingt-quatre semaines. Les points les plus couramment utilisés : baihui (VG20) et zusanli (ES36) avec une incidence de 40% parmi ces 10 ECR et Xuehai (RA10) et Sishencong (EX1) dans 30% des 10 ECR. Les autres points utilisés : taixi (RE3), dazhui (VG14), danzhong (VC17), zhongwan (VC12), qihai (VC6), waiguan (TR5), xuanzhong (VB39), neiguan (MC6), shenshu (VE23), yintang (VG29), sanyinjiao (RA6) et dazhong (RE4), ainsi que la craniopuncture et deux points d’acupuncture expérimentaux "xiu sanzhen" et "si shenzhen".

Les résultats combinés de six essais ont objectivé que, versus le traitement médicamenteux, l’acupuncture améliore de façon statistiquement significative les résultats sur l’échelle Mini Mental State Examination (MMSE) avec une différence moyenne standardisée à modèle aléatoire (SMD) égale à 1,05 (IC 95%, 0,16 à 1,93). Cependant, il existe une grande hétérogénéité des six ECR (I²  = 57% ; p=0,04). De même, les résultats de trois ECR montrent que l’acupuncture associée au donépézil est plus efficace que le donépézil seul dans l’amélioration du score MMSE (SMD à modèle fixe = 2,37 ; IC 95% : 1,53 à 3,21 ; pas d’hétérogénéité (I²  = 11% ; p=0,32). Sur cent-quarante et un essais cliniques, deux études ont rapporté des effets indésirables liés à l’acupuncture. Sept des 3416 patients ont eu des effets indésirables liés à l’acupuncture pendant ou après le traitement mais les réactions ont été décrites comme tolérables et bénignes.

En conclusion, en terme d’amélioration de la fonction cognitive, l’acupuncture peut être plus efficace que les médicaments et peut même augmenter leur effet dans le traitement de la maladie d’Alzheimer. L’acupuncture peut également être plus efficace que la thérapeutique usuelle dans l’amélioration de la capacité des patients à mener à bien leur vie quotidienne. En outre, l’acupuncture est sans danger. Néanmoins, il est nécessaire de confirmer ces différentes données par des ECR de meilleure qualité méthodologique, avec un processus de randomisation et de mise en insu de la répartition de la population étudiée plus rigoureusement contrôlé et décrit. Il est également nécessaire que ces ECR offrent davantage d’informations concernant le diagnostic de la maladie d’Alzheimer avec une mesure du degré de volume d’atrophie hippocampique mesurée par IRM. Enfin la population étudiée doit être plus importante.
